Trinity Bridge’s Virginia Edley, a prominent industry reliability and inspection consultant, has published an important article on PEI incident investigations in the September/October 2015 issue of Inspectioneering Journal. Ms. Edley uses the analogy of the U.S. Department of Homeland Security’s directive ‘If you see something, say something’ to explain how a similar vigilance in investigating small incidents at the plant can prevent major failures.
Ms. Edley was the Master Editor for the American Petroleum Institute’s recommended practice, API RP 585, Pressure Equipment Integrity Incident Investigation. She has extensive experience in the oil and gas industry in upstream production, petrochemical plants and refineries.
